The Food Bank gives away 15 tons of food each week to agencies in the local Fairbanks community that then use the food for their own programs.

Annual Agency Monitoring Form
Agencies interested in participating in the Agency Shopping program must complete the Agency Monitoring Form.
A copy of each agency’s IRS Letter of Determination for 501(c)(3) and a letter on the Agency’s letterhead signed by the director/administrator/pastor must also be turned in with the form. The Fairbanks Community Food Bank’s IRS Letter of Determination for 501(c)(3) is included in the PDF packet as a sample of the specific form that we need you to submit.
